Social Care Network (SCN) provides comprehensive support for pregnant and postpartum individuals.

How We Help

Through SCN, every client is screened to better understand their needs and then connected to essential community resources.

Our screening process looks at key areas that affect health and well-being, including:

• Housing
• Food
• Transportation
• Employment
• Education
• Physical Impairment
• Domestic Violence

The SCN focuses exclusively on the pregnant and postpartum community. We also ask about additional needs related to pregnancy, such as access to baby supplies.

Important: We support all pregnant and postpartum individuals regardless of insurance status.
