Brooklyn Healthy Start Infant Mortality Reduction Initiative
Outreach, case management and follow-up services for pregnant women and those parenting infants up to two years old. Moderate to high risk women are referred for case management services appropriate to need.

Who is Eligible?
Brooklyn Residents who are pregnant and/or are parenting infants or young children are eligible. This is a collaborative project with several other Brooklyn community-based organizations that participate on the Brooklyn Task Force on Infant & Maternal Mortality & Family Health.
