Brooklyn Healthy Start Infant Mortality Reduction Initiative
Outreach, case finding and follow-up services for pregnant women and those parenting infants up to two years old. Moderate to high risk women are referred for case management services appropriate to need.

Child Health Plus New York State has a health insurance plan for kids, called Child Health Plus. Depending on your family’s income, your child may be eligible to join either Child Health Plus A (formerly Children’s Medicaid) or Child Health Plus B. Both Child Heath Plus A and B are available through dozens of providers throughout the state. Enrolling in Child Health Plus is easy.

Family Health Plus
Family Health Plus is a public health insurance program for adults between the ages of 19 and 64 who do not have health insurance — either on their own or through their employers — but have incomes too high to qualify for Medicaid. Family Health Plus is available to single adults, couples without children, and parents with limited income who are residents of New York State and are United States citizens or fall under one of many immigration categories.

Medicaid
Medicaid is a program that pays for medical assistance for certain individuals and families with low incomes and resources. This program became law in 1965 and is jointly funded by the Federal and State governments (including the District of Columbia and the Territories) to assist States in providing medical long-term care assistance to people who meet certain eligibility criteria. Medicaid is the largest source of funding for medical and health-related services for people with limited income.

Brooklyn Infant Mortality
Reduction Initiative (BIMRI) Outreach and case finding; referrals to other organizations for health, case management and support services, school and community health presentations and information sessions; and community health awareness events.
Who is Eligible?
Brooklyn Residents who are pregnant and/or are parenting
infants or young children are eligible. This is a collaborative
project with several other Brooklyn community-based
organizations that participate on the Brooklyn
Task Force on Infant & Maternal Mortality &
Family Health.

Sister Friend Birthing Project
Using the extended family model, this project pairs a trained volunteer (Sister Friend) to a pregnant teen or woman (little sister). The Sister Friend provides support during the pregnancy up to the baby's first birthday, while encouraging the utilization of adequate and appropriate health care services and follow-up for mother and the baby.
BPN's Baby Buggy Program
Pregnant and parenting women who participate in BPN's programs can receive infant & Child supplies, clothes & furniture, if available. Some items include diapers, bibs, clothing, books, toys and strollers.
Some items for mom are also available.
